Are you living with heart failure? 

The HeartFailureHospital@Home app gives you hospital care in the comfort of your own home. This is also known as a Virtual Ward.  

It’s a safe and effective option for some eligible heart failure patients that would normally need a hospital stay. 

Being at home means better rest, less stress, and a lower risk of complications like infections. You can stay in your own routine, surrounded by loved ones, with care tailored to your needs. 

If you have been referred to this service, a consultant has assessed your condition and feels that it is safe for your care to continue to be delivered at your usual home.

Why use the HeartFailureHospital@Home service? 

  • The service provides safe, effective care, much like the care you would receive in hospital, but in the comfort of your own home 

  • Blood pressure measurements and other readings and treatments can easily be delivered at home by our specialist teams  

  • You will have a personalised care and support plan to meet your needs 

  • Your heart failure care team will work with you to better understand your condition and help you learn how to manage your heart failure and stay well  

How do I register for the service? 

While HeartFailureHospital@Home has many benefits, it’s not suitable for everyone. Your healthcare team will assess if your condition is stable enough to be treated safely at home. Your home also needs to be a safe and comfortable place for care. 

The time that each patient spends under this service will vary, depending upon their health and care needs. This will be discussed with patients if they are suitable and will be updated as their condition changes.  

Patient safety

  • Daily contact: Nurses will contact you every day (weekdays only) to check your health and adjust your care as needed. 

  • 24/7 Support: You will have a dedicated phone number to call anytime, day or night, if you need urgent help or advice. If your condition changes or becomes more serious, the team will arrange for quick transfer to hospital.

  • Safe Equipment: All medical devices and treatments used at home meet high safety standards and are carefully managed.

  • Clear Communication: The care team keeps in touch with your GP and specialists to coordinate your treatment. 

  • You will be given all the important contact numbers and information before your care begins. If you ever feel unsure or worried, don’t hesitate to reach out — we’re here to support you every step of the way. 

  • Your care team will explain everything and answer any questions, so don’t hesitate to ask. 

FAQs

Hospital at Home is safe, but if your condition changes suddenly, you may need to go to the hospital quickly.

Yes, your care team will manage all your treatments together. 

It depends on your needs—contact will be daily (Monday-Friday) or less often we out of hours number if you need advice. 

Refer to terms of service within the app for information on how your data will be used. 

Your GP does not require access to the app data. As with ‘usual care’, heart failure reviews and treatment will be shared to the GP clinical record and changes to medications will be communicated to the surgery. 

You might decide, with your heart failure care team, that you no longer need to use the app, or that you would like to take a break or stop using it altogether. You should speak to your heart failure care team about returning to ‘usual care’. 

If you would like to go back to using the app in the future, and it’s still suitable for managing your care, you can ask your care team to re-register you.
